State Required Rec Kids Forms
Our Rec Kids Day Camps are required to be licensed by the State of Colorado just like public schools and daycare facilities. The state required forms must be filled out every year and signed by a legal guardian and approved by our staff before you can register your child for Rec Kids.
State Forms for the 2024-25 school year are available below. All state forms in our system will expire on August 31, 2025. In order to register for school year programming, you will need to submit updated forms. We are moving our state forms to our SmartRec registration platform. Paper forms will not be accepted.
Please note that we need a physical address for all emergency contacts, authorized pick-ups, and medical providers.

Once you submit forms please allow up to 4 business days for our staff to approve the forms and lift the registration block on your account in SmartRec, our registration software.

Why Do I Have to Fill These Out?
These forms help ensure the safety of your child in emergency situations and inform our staff of any special health or behavioral concerns. In order to keep our childcare license valid with the state, we MUST have a copy of these forms on-hand at every camp we offer for every kid at camp that day. If we break this rule, we risk losing our license and the ability to provide these important programs for our community.How Often to I need to fill these out?
We need these forms annually. Forms will expire on August 31st every year. So forms are valid from September 1st through August 31st of the following year. We suggest submitting these forms in September because the immunization form you provide Eagle County Schools is the same form we need. Submitting in September also gives you time to submit these forms in the break between summer Rec Kids and school year Rec Kids ensuring you have no gaps in childcare coverage.When Should Parents Submit Forms?
As early as possible, we do not recommend turning in forms and trying to register the same day of camp, this can cause frustration if the forms are not complete or require a doctor’s signature. Our camps fill up fast so getting these forms done ahead of time gives you the best chance of signing up.Incomplete forms will not be accepted due to state laws, please ensure you fill these forms out completely.
